RN - Patient Care Coordinator - University Med/Surg - FT Nights

RN - Patient Care Coordinator provides patient care outcomes through the coordinated delivery of nursing care for designated areas. Under guidance from the Clinical Director, deploys priorities for clinical quality, service and Associate development and retention. Ensures a collaborative plan of care is implemented by Registered Nurses and other team members; assists professional nurses with coordination of patient care with the multidisciplinary team; ensures responsiveness to physicians; directs on-site coaching and staff assignments; conducts Associate performance evaluations in collaboration with the Clinical Director; maintains readiness for accreditation and regulatory requirements; oversees upkeep of facilities and patient care equipment. Makes rounds to ensure care is delivered per plan; orchestrates patient flow and bed assignments; supports safety, discharge planning, and resource management; provides clinical leadership and feedback to staff; participates in quality reviews; collaborates with Case Managers and other departments to ensure timely services and appropriate patient movement.
