Job Summary
Provide comprehensive patient assessments and develop individualized care plans; administer medications and treatments as prescribed; monitor patient progress and adjust care plans; educate patients and families on health management; collaborate with a multidisciplinary team; maintain accurate EHR documentation; assist in training and mentoring new nursing staff and caregivers; participate in quality improvement initiatives; requires valid GA RN license and minimum 2 years home health experience; CPR/First Aid certification; strong assessment, communication, and teamwork skills; flexible work-from-home options available.
Required Qualifications
- Valid Registered Nurse (RN) license in the state of Georgia
- Minimum of 2 years of nursing experience in home health or related fields
- Current CPR and First Aid certification
- Proficient in electronic health record (EHR) systems
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
