RN Palliative Care Coordinator - West Penn - Full Time (Hybrid)

Assess patient physical, psychosocial, and economic needs to facilitate effective transition of care planning. Contribute to developing transition plans for patients and families while documenting and validating data to monitor interventions and outcomes. Coordinate care between Highmark entities including Endorsed LLC, Healthcare@Home, and Journeys, and participate in Goals of Care conversations with patients or designated proxies. Perform symptom assessment, management, and med titration using the nursing process to implement individualized patient plans of care. Participate in department-specific quality initiatives, ensure appropriate order sets are used, and integrate performance improvement principles into all job responsibilities. Full-time hybrid role requiring a Bachelor's Degree in Nursing and current PA RN licensure. Salary range $55,000-$65,000. Located in Pittsburgh, PA.
