Job Summary
Coordinate medical care for assigned individuals, monitor and assess health needs including arranging for follow-up care. Perform direct observation of consumer health status, evaluate physical status changes, and document all findings. Communicate with physicians, families, and interdisciplinary team members regarding hospitalizations, injuries, or distress. Develop individual nursing plans and protocols, and supervise AMAP administration of prescribed medications and treatments. Participate in meetings with management, staff, and clinicians to discuss health issues. Be available for urgent phone calls outside routine hours. Requires current NY State RN license, CPR certification, and ability to handle multiple priorities.
Required Qualifications
- Must be currently licensed as a Registered Professional Nurse (RN) in New York State
- CPR certification required
- Must demonstrate excellent written and oral communication skills
- Ability to handle multiple priorities and work as a member of a team
Desired Qualifications
- Experience working with individuals with developmental disabilities preferred
- A valid NY State or Tri-State Driver's License for more than one year, with a good driving record Preferred
