Job Summary
Coordinate medical care for assigned individuals, monitor and assess health needs including arranging for follow-up care, and evaluate physical status changes for exacerbation of existing conditions. Conduct direct observation for signs of distress or illness, document findings, and communicate with physicians, families, and interdisciplinary team members regarding hospitalizations, injuries, or health issues. Administer prescribed medications and treatments, supervise AMAP administration, and develop individual nursing plans and protocols. Attend meetings with management, staff, and clinicians to discuss consumer health issues. Handle urgent calls outside routine hours as needed. Requires current NY State RN license, CPR certification, and ability to work flexible per-diem schedules of 10–16 hours per week across three Bronx locations.
Required Qualifications
- Must be currently licensed as a Registered Professional Nurse (RN) in New York State
- CPR certification required
- Must demonstrate excellent written and oral communication skills
- Ability to handle multiple priorities and work as a member of a team
- Must be able to lift 50 lbs
Desired Qualifications
- Experience working with individuals with developmental disabilities preferred
- A valid NY State or Tri-State Driver's License for more than one year, with a good driving record Preferred
