RN Outpatient Cardiology

Assess and monitor patient needs, document care plans, and deliver culturally sensitive nursing care consistent with ANA standards and physician orders. Initiate discharge planning, facilitate smooth transitions, and advocate for patient autonomy while communicating with the health care team to ensure continuity. Evaluate outcomes of nursing interventions, develop individualized care plans, and participate in quality improvement activities using clinical paths. Maintain ethical behavior and data security while delegating tasks to support staff. This role supports Corewell Health's mission of equitable care across 22 hospitals and 300+ outpatient locations.
