RN or LPN-Outpatient-LCMC

Conduct walk-in and telephone triage, coordinate patient care through collaboration with the patient/family and healthcare team, and assess, plan, implement, evaluate, and document individual patient needs. Manage case management based on PCP panels in collaboration with network providers, including nurse visits and telephone management. Delegate aspects of patient care to healthcare team members commensurate with their validated competency and ensure documentation within scope of practice. Perform other functions as required.
