RN-Oncology Nurse Navigator

Serve as the primary point of contact for newly diagnosed oncology patients, guiding them through diagnosis, treatment planning, and follow-up care. Coordinate appointments, diagnostic testing, consultations, and treatment schedules while identifying barriers to care and bridging gaps in transitions. Collaborate with multidisciplinary teams to implement tumor board recommendations, monitor patient progress, and facilitate referrals to specialists and supportive services. Provide individualized education on diagnosis, treatment options, symptom management, and resources, while advocating for patient preferences and goals throughout the journey. Perform nursing assessments, triage concerns, and promote adherence to treatment plans in accordance with clinical protocols. Maintain current knowledge through continuing education and participate in multidisciplinary conferences and tumor boards.
