RN On Call - Weekends

Registered Nurse On Call for weekends (Friday 8am–8pm and Saturday 8am–8pm). Provide home visits for patient assessment, treatment, and support, develop and revise nursing care plans with the patient’s physician, communicate changes to the care team, ensure timely documentation, and perform duties to comply with Medicare regulations and facility policies. Role emphasizes compassionate, patient- and family-centered hospice care, with emphasis on crisis management, pronouncement of death when applicable, and coordination of care across the IDG.
