RN On-Call

Provide home visits for patient assessment, treatment, and support, including pronouncement of death, crisis management, and admissions. Develop and revise nursing care plans with physicians and IDG, prepare clinical progress notes, and submit all documentation timely. Inform physicians and personnel of patient needs changes and intervention outcomes while evaluating family responses. Ensure compliance with local, state, and federal laws, Medicare regulations, and personnel policies. Utilize proper phone etiquette with internal and external customers.
