Job Summary
Coordinate and manage patient care from pre-op through recovery and follow-up for orthopedic surgery patients. Provide patient and family education, conduct inpatient rounding, and assist with discharge planning. Collaborate with physicians, nurses, and support services to monitor quality, patient outcomes, and length-of-stay metrics. Maintain accurate documentation in EPIC and utilization management systems while serving as a clinical resource and mentor to staff. Support program growth, physician relationships, and community outreach. This role facilitates the new care model, ensuring timely, safe, and effective care for a 901-patient population in the Washington, MO specialty clinic.
Required Qualifications
- Graduate of an accredited nursing program
- Current RN license
- BLS certification
- 2–3 years of nursing experience
Desired Qualifications
- BSN
- ACLS certification
- Orthopedic, care coordination, navigator, or case management experience
