Job Summary
Lead, mentor, and support caregivers and field staff in delivering compassionate, dependable care for adult and senior clients. Conduct initial and ongoing client assessments, supervisory visits, and service check-ins while overseeing care planning, documentation, and communication with families. Supervise Home Health Aides through orientation, training, and skills validation, and help coordinate schedules to ensure continuity of care. Partner with leadership on quality, compliance, and client satisfaction, responding to changes in client needs to keep care plans accurate and timely. This role combines office-based leadership with field-based client visits, requiring occasional local travel for supervisory visits and community engagement.
Required Qualifications
- Current Registered Nurse (RN) license
- At least 2 years of RN experience
Desired Qualifications
- Home care, assistive care, personal care, or supervisory experience preferred
- Strong leadership, communication, coaching, and relationship-building skills
- Comfort balancing office responsibilities with field-based client visits
- Ability to manage competing priorities while maintaining strong attention to detail
- A genuine passion for helping clients live safely and with dignity at home
