Job Summary
RN Nurse Home Visitor (NFP) provides nursing care and education to women and families in the home setting as part of the Nurse-Family Partnership model. Responsibilities include comprehensive assessments of physical, emotional, social, and environmental needs; development and implementation of individualized care plans; data collection and documentation per NFP guidelines; educational support and referral resources to help families achieve targeted goals; and upholding MLH mission and values. Requires professional nursing expertise, adherence to NFP guidelines, and ability to travel to educational sessions in Denver, CO while serving Memphis/Mid-South communities.
Required Qualifications
- Bachelor's Degree in Nursing
- Registered Nurse Tennessee license
- Basic Life Support (American Heart Association)
- Strong knowledge of nursing process and clinical nursing practice
- Experience or exposure to maternal/child health, home visiting, or pediatric nursing (preferred)
