Job Summary
Oversee a shared caseload with LPN Case Managers by conducting regular residence visits to identify and provide plans of care for individuals served. Review and document nursing plans monthly, after hospitalizations, or as needed, while attending specific medical appointments and facilitating health education for persons, caregivers, and staff. Identify non-compliance areas, report to leadership, and advocate for individual rights in full compliance with MAP regulations and agency policies. Maintain confidentiality under HIPAA while managing multiple priorities, travel within 100 miles of the Sturbridge office, and participate in monthly nursing team meetings and staff training.
Required Qualifications
- Current and valid registration as a professional nurse under the Massachusetts Board of Registration in Nursing
- At least one year of full time working experience as an LPN
- Must be 21+ years old
- Have a valid driver's license
- Daily use of a car on site
- Available by telephone for daily contact
- Employee must possess the knowledge; skills and abilities listed above
- Demonstrate that they can perform the essential functions of the job
- Employee must be able to speak, read and write in fluent English
- Licensed as a Registered Nurse in Massachusetts by the Board of Registration
- Two years of experience as a registered nurse
- Must be 21+ years old
- Have a valid driver's license
- Daily use of a car on site
- Available by telephone for daily contact
- Must be able to bend, lift, stoop, reach and file
- Maintain CPR, AED & Safety Care certifications
- Complete Agency Nursing Orientation
Desired Qualifications
- Experience with the developmentally disabled population preferred
