Job Summary
Provide professional nursing care through telephonic interventions focused on the patient and caregiver as a whole. Manage patient needs through transitional care while demonstrating commitment to Baptist Health's quality standards, organizational values, and safety protocols. This remote position requires residency in KY or IN, with shifts scheduled 8 hours per day from 8:00am to 4:30pm EST. The role supports clinical excellence within a health system dedicated to improving community health across Kentucky and Indiana.
Required Qualifications

- Residency in KY or IN

- Associate degree in Nursing
- Five years of experience
