Assess patient physiologic and psychosocial health status through interviews, chart reviews, and clinical observations across gastrointestinal, cardiovascular, and neurological systems. Develop individualized plans of care, prioritize interventions, and implement treatments including IV drips, medication administration, and grade I-IV pressure injury care. Collaborate with physicians and care team members to coordinate procedures, delegate assignments, and manage emergency situations while ensuring safety precautions are followed. Document all assessments, treatments, and outcomes accurately per departmental policy. Perform patient and family education on conditions, drug interactions, and discharge planning. Schedule includes every other weekend and holidays.
