Job Summary
Provide direct home-based nursing care for ESRD patients, leading the coordination of scheduling, training, clinic visits, and monthly QA/Ops meetings. Deliver care per nursing practice guidelines, document per CDC standards and EMR timelines, and ensure interdisciplinary communication. Supervise and mentor RN/LPN/PCT staff, oversee patient training for home care modalities, coordinate with MD for new patient training and transfer, and ensure patient competency and adherence to dialysis treatment plans. Maintain aseptic technique, medication administration, patient confidentiality, and policy procedure updates; contribute to policy development and continuous quality improvement. Requires nursing degree and active Ohio licensure; CPR; strong leadership and collaboration skills.
Required Qualifications
- Graduate from an accredited school of nursing
- Current Ohio licensure
- Current CPR certification
- Independent decision-making skills
- Good leadership and interpersonal qualities
- Home Care in the ESRD field preferred
