Job Summary
Conduct after-hours field visits to patients in homes, assisted living facilities, nursing homes, or hospitals to assess immediate needs and manage direct care within an interdisciplinary group. Provide compassionate customer service, educate families on disease processes and palliative interventions, and initiate skilled nursing interventions for symptom management and comfort. Collaborate with physicians and case managers to develop plans of care, obtain informed consents, and facilitate safe discharges while documenting all interactions per policy. Handle emergencies by reporting to designated locations for continuity of services, including overnight stays during lockdowns. Maintain professional relationships with referring hospitals and community partners to ensure timely follow-up and referral coordination.
Required Qualifications
- Current license as RN in the state where the employee will be working
- Minimum of one (1) year nursing experience
- Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system
- Ability to effectively communicate in English (verbal and written)
- Valid driver's license
- Automobile insurance per Company policy
- Reliable transportation to meet patients' needs
- Active BLS for healthcare professionals from the American Heart Association or Red Cross
- Ability to perform sedentary work with frequent interruptions
- Ability to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodations
- Ability to work in a cooperative team environment
- Ability to exhibit a sense of urgency in daily work activities
- Skilled in computer operations
- Proficiency in MS Office software applications
- Proven data entry and navigation skills
- Available to work shifts to accommodate extended hours of operation as scheduled

- Consent to drug and/or alcohol testing after a conditional offer of employment is made

- Bi-lingual in Spanish
