Job Summary
RN in Nephrology Clinic providing evidence-based nursing care via telephone and MyChart; assist with care coordination, patient education, prior authorizations, and duties supporting the Nephrology/PKD Center of Excellence; manage referrals, triage, and transitions to dialysis; educate Chronic Kidney Disease patients to delay progression; work within a multidisciplinary team to optimize patient outcomes; determine patient care needs using the Nursing Process. The role is 90% FTE, day shift, Monday–Friday, in Fitchburg and Madison, WI.
Required Qualifications
- Graduate of school of nursing
- Registration as a registered nurse in the state where employed or licensure in a state in the licensure compact Upon Hire Required
- CPR/BLS Certification Upon Hire Required
- 6 months of RN experience Required
- Applicable clinical certification Preferred
- Bachelor's Degree in Nursing (BSN) Preferred
