Job Summary
RN Navigator, Pediatric Complex Care Clinic at UW Health (American Family Children’s Hospital) in Madison, WI: provide evidence-based nursing care and care coordination for pediatric patients with multiple chronic health conditions and acute episodes via telephone and MyChart; collaborate with a multidisciplinary team; educate patients and families; support prevention and wellness; involvement in clinic procedures and coordination to achieve positive patient outcomes; 8:00am–5:00pm, Monday–Friday (100% FTE) with no weekends or holidays; BSN required, Master’s degree preferred, 3 years of clinical nursing experience required, Wisconsin RN license and BLS/CPR required, oncology certification preferred.
Required Qualifications
- Bachelor's Degree in Nursing (BSN) Required
- Master's Degree in Nursing, Social Work, Public Health, Healthcare Administration, or related field Preferred
- 3 years of relevant clinical nursing experience Required
- 1 year of relevant experience in the specific patient population being supported Preferred
- Registered Nurse license in the state of Wisconsin Upon Hire Required
- Basic Life Support/CPR Upon Hire Required
- Applicable Oncology certification based on patient population Preferred
