Job Summary
Coordinate patient care across the cancer continuum by serving as the primary advocate, conducting assessments, and eliminating barriers to timely delivery of services. Provide education and support to patients, families, and the multidisciplinary team regarding treatment options, symptom management, and survivorship. Act as a liaison to facilitate communication, schedule appointments, and ensure adherence to treatment plans while managing referrals to specialists and community resources. Document all encounters comprehensively, participate in tumor boards, and track program metrics to drive quality improvement initiatives.
Required Qualifications
- Basic Life Support (BLS) within 30 days - American Heart Association (AHA)
- Licensed Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR)
- College Diploma: Nursing
- Possesses clinical trial awareness
- Understands criteria for molecular testing and genetic counseling
- Possesses a basic understanding of insurance (co-pays, deductibles, co-insurance)
- Maintains/uses a comprehensive database of local, regional and national resources
- Understands resources available to patients experiencing financial hardships and/or are uninsured/underinsured
- Exhibits customer service skills, leadership skills, takes initiative and is self-directed, cultural sensitivity/language skills, patient triage skills, nursing theory and practice knowledge and skills, problem-solving skills, and advocacy skills
- Performs the functions of assessing, planning, implementing and evaluating the care delivered according to the nursing process and Oncology Nursing Society Standards of Practice
