RN Navigator - Oncology

The RN Navigator - Oncology provides care management support to a panel of patients requiring specialty services, collaborating with the interdisciplinary care team to develop, implement, monitor, and update the plan of care across the care continuum. Responsibilities include coordinating services, gathering data, educating patients and families, assessing needs, and facilitating communication among patients, families, and healthcare providers. This role leads transition and discharge planning, supports research and program development, participates in quality improvement activities, and may travel to multiple locations in support of patients and department. A hybrid work arrangement is available; mileage reimbursement is eligible, and 1.0 FTE (40 hours per week) with day shifts and no weekend requirements are described. Licensure as a Registered Nurse in MN (and WI if applicable for in-state Wisconsin patients) is required; the role requires careful collaboration with care teams, adherence to evidence-based practice, and the ability to navigate complex care plans within a system of care. The pay range is described as $44.64 to $61.76 per hour, with potential shift differentials.
