RN Navigator – Neurology

RN Navigator – Neurology role provides care management support to a panel of patients requiring specialty neurology services (movement disorders and general neurology). The nurse interacts with specialty and primary care teams to assess, plan, implement, coordinate, monitor, and evaluate the plan of care, leveraging guidelines, coordinating with external resources, documenting outcomes, and supporting transitions across care settings. Responsibilities include patient and family education, coordination of services and benefits with insurers, ensuring continuity of care across the continuum, leadership in quality improvement activities, and potential travel to multiple locations for care delivery. Requires ability to work with an interdisciplinary team and perform navigation across the care continuum, including discharge planning and program development. The role is a full-time position with day shifts (7:30am-4:00pm) at United Hospital in Saint Paul, MN, with potential to float to ANW and WestHealth clinics and eligibility for float pay and mileage reimbursement.
