Job Summary
Provide continuity of care by ensuring smooth transitions between care settings and managing high-risk, complex patients to minimize readmissions. Act as a liaison and communicator with patients, caregivers, physicians, and multi-disciplinary teams to facilitate discharge planning and navigate the healthcare continuum for cost-effective outcomes. Coordinate patient care plans, identify barriers to care, and collaborate with leadership to improve clinical experiences for referred patients. Adapt communication styles to specific patient populations while maintaining compliance with federal, state, and local laws and accreditation standards. Requires a current RN license, BLS certification, and 2 years of nursing experience; involves flexible scheduling including weekends, holidays, and on-call availability.
Required Qualifications
- Graduate of an accredited school of nursing
- 2 years of nursing experience
- Current registered nurse (RN) license in state of practice
- Basic Life Support (BLS) from the American Heart Association
- Proficiency in using computers, software, and web-based applications
- Effective verbal and written communication skills
- Ability to work a flexible work schedule (e.g. 24/7, weekend, holiday, on call availability)
- Ability to travel throughout and between facilities
- Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects
- Exposure to blood, body fluid and tissue
- Exposure to all communicable diseases
- Exposure to hazardous medications or hazardous waste
Desired Qualifications
- Associate's or Bachelor's degree in nursing
- Certification in clinical specialty area
