Coordinate appointments, services, and resources while providing education and emotional support to patients and families navigating complex care pathways. Serve as the primary point of contact within the Healthcare & Medical Services department, collaborating closely with physicians and the interdisciplinary team to promote continuity of care and remove barriers to treatment. This role focuses on guiding individuals through specific care processes to improve patient outcomes within a compassionate, learning-focused culture.
