Job Summary
The RN Navigator serves as a liaison, educator, and care coordinator across CHWC sites within the ACO framework. Responsibilities include patient navigation across the continuum of care, identifying and addressing care gaps, coordinating admissions/discharges/referrals, supporting CCM programs, monitoring quality metrics, performing RN triage (walk-in and telephone), and providing clinical care within licensure scope. Requires cross-site float coverage, strong communication, data documentation in EHRs, and collaboration with interdisciplinary teams to improve population health outcomes.
Required Qualifications
- Current Registered Nurse (RN) license in the State of Connecticut
- BSN preferred
- Current CPR certification
- Graduate of an accredited nursing program
