Job Summary
Provide continuity of care by ensuring smooth transitions between care settings and managing high-risk, complex patient care to minimize readmissions. Discuss alternative care options with patients and caregivers while assisting with discharge planning needs and navigating the healthcare continuum. Collaborate with physicians and multidisciplinary teams to maintain coordinated, patient-centered care plans and identify barriers to care for process improvement. Prepare and execute post-discharge care plans while adapting communication styles to specific patient populations. Requires a current RN license, two years of nursing experience, and BLS certification. Full-time role at Ochsner Health, a leading not-for-profit provider recognized as the No. 1 hospital in Louisiana.
Required Qualifications
- Registered Nurse Diploma
- Associate's or Bachelor's degree in Nursing
- 2 years Nursing experience
- Current RN License in the state of practice
- Basic Life Support (BLS) from the American Heart Association
- Computer skills and dexterity required for data entry and retrieval of patient information
- Effective verbal and written communication skills and the ability to present information clearly and professionally to varying levels of individuals throughout the patient care process
- Must be proficient with Windows-style applications, various software packages specific to role and keyboard
- Excellent conflict resolution skills
- Frequently exerting 10 to 20 pounds of force to move objects
- Occasionally exerting up to 100 pounds of force
