Job Summary
Coordinate and complete accurate Minimum Data Set (MDS) assessments for residents, leading the Resident Assessment Instrument (RAI) process and translating clinical findings into precise MDS documentation. Collaborate with an interdisciplinary team to inform individualized, person-centered care plans, monitor outcomes, and promote regulatory compliance. Educate staff on MDS requirements and changes in regulations; participate in care conferences and contribute to care plan development, audit readiness, and continuous quality improvement. Primarily on-site in Clark, SD, with limited remote work for documentation review and data entry as operational needs permit.
Required Qualifications
- Registered Nurse (RN) license
- MDS knowledge/experience
- RAI process lead/coordination
- CMS regulations and state survey standards familiarity
- EHR proficiency
- strong assessment and documentation skills
- ability to collaborate with interdisciplinary teams (physicians, therapy, social work, nursing)
- quality improvement and audit readiness experience
