Job Summary
RN MDS nurse to coordinate and complete accurate MDS assessments for residents, leading the Resident Assessment Instrument (RAI) process, gathering clinical information from the interdisciplinary team, reviewing medical records, and translating clinical findings into MDS documentation to support person-centered care plans and regulatory compliance. Responsibilities include educating staff on MDS requirements, participating in care conferences, contributing to care plan development and review, monitoring resident outcomes, ensuring audit readiness, and supporting continuous quality improvement. Requires strong assessment skills, critical thinking, CMS/state survey regulation knowledge, chart review, data analysis for quality indicators and case mix, and effective communication with diverse teams and families. On-site with limited remote work possible for documentation review and data entry. EHR experience and a commitment to serving vulnerable populations are essential.
Required Qualifications
- Registered Nurse (RN) license
- Knowledge of Minimum Data Set (MDS) and CMS regulations
- Experience with electronic health records (EHR) and documentation
- Ability to coordinate and complete MDS assessments and support care planning
