Job Summary
RN MDS nurse responsible for coordinating and completing accurate MDS assessments, leading the RAI process, translating clinical findings into MDS documentation, and supporting person-centered care plans. Collaborates with interdisciplinary teams, ensures regulatory compliance with CMS/state standards, contributes to quality measures and reimbursement accuracy, and supports care conferences and continuous quality improvement. Primarily on-site with limited remote work available for documentation review/data entry; strong EHR proficiency and ability to advocate for vulnerable populations are essential.
Required Qualifications
- Active RN license
- MDS expertise and experience with Resident Assessment Instrument (RAI) process
- Experience interpreting CMS regulations and state survey standards
- Proficiency with electronic health records (EHR)
- Strong assessment and critical thinking skills
- Ability to coordinate interdisciplinary teams (physicians, therapy, social work, nursing staff)
- Excellent communication with diverse teams and families
- Ability to manage multiple deadlines and maintain audit readiness
- Knowledge of care planning and quality improvement processes
