Job Summary
RN MDS nurse coordinated and completes accurate MDS assessments, leads the RAI process, translates clinical findings into precise MDS documentation, and supports individualized care plans reflecting residents’ physical, emotional, social, and spiritual needs. Works with the interdisciplinary team to ensure regulatory compliance, quality measures, and audit readiness, shaping care plans and resident outcomes. Primarily on-site with possible limited remote work for documentation review and data entry, requiring strong assessment skills, CMS/state regulation knowledge, and proficiency with electronic health records.
Required Qualifications
- Active RN license
- Experience with Minimum Data Set (MDS) assessments
- Knowledge of Resident Assessment Instrument (RAI) process
- Familiarity with CMS regulations and state survey standards
- Ability to review charts and validate clinical documentation
- Interdisciplinary collaboration skills (physicians, therapy, social work, nursing)
- Report and data analysis for quality indicators and case mix
- On-site role with potential for limited remote work for documentation review and data entry
