Job Summary

Coordinate and complete accurate, timely Minimum Data Set (MDS) assessments for residents, ensuring regulatory compliance while promoting meaningful health and wellness outcomes. Lead the Resident Assessment Instrument (RAI) process by gathering clinical information from the interdisciplinary team, reviewing medical records, and collaborating with physicians, therapy, social work, and nursing staff to translate findings into precise documentation. Educate staff on MDS requirements, changes in regulations, and best practices in documentation. Participate in care conferences, contribute to the development and review of care plans, and monitor resident outcomes to ensure care is person-centered. Review charts, validate clinical documentation, and analyze data related to quality indicators and case mix to support audit readiness and continuous quality improvement initiatives.
