Job Summary
Coordinate and complete accurate, timely Minimum Data Set (MDS) assessments for residents; lead and manage the Resident Assessment Instrument (RAI) process, translating clinical findings into precise MDS documentation to support individualized care plans; collaborate with physicians, therapy, social work, and other nursing staff; monitor resident outcomes and contribute to quality measures, reimbursement accuracy, and audit readiness; educate staff on MDS requirements and changes in CMS regulations; participate in care conferences and care-plan development; work with leadership on continuous quality improvement while maintaining compliance; primarily on-site with limited remote documentation review and data entry as needed.
