Implement and monitor patient care plans, record and communicate patient conditions, and coordinate all disciplines for well-coordinated care. Carry out physician and nursing orders while assessing and coordinating discharge planning needs with the healthcare team. Serve as the primary point of contact for patients diagnosed with pneumonia, COPD, sleep apnea, or medical telemetry across a 23-bed unit. Work Monday through Friday in a 18:45-07:15 shift covering 36 hours per week with a 6:1 nurse-to-patient ratio. Maintain BLS and Ascension MOAB certifications.
