Job Summary
Coordinate holistic healthcare for children in the Baltimore City Department of Social Services (BCDSS) foster care, ensuring timely access to medical and mental health care. Collaborate with foster parents, biological parents, managed care organizations, and healthcare providers; assess and monitor medical needs; identify immediate needs at registration; review initial medical documentation; make recommendations for continuing care teams; participate in weekly case transfer meetings; provide resources and education to caregivers and child welfare workers; assist families in navigating Health Choice to establish a medical home; prioritize care based on urgency; document assessments, medications, and treatment plans in relevant databases; attend weekly team and inter-agency meetings; complete required trainings; may work beyond standard hours. The role requires strong organizational, communication, problem-solving skills, and cultural sensitivity in a nursing-focused care coordination context.
Required Qualifications
- A bachelor’s degree in nursing from an accredited college/university
- Current licensure as a Registered Nurse in the State of Maryland.
- One year of clinical nursing experience.
- Relevant clinical experience providing services to families and children, preferably in a public health/community setting.
