Job Summary
RN MDS Nurse / Resident Assessment Coordinator (RAC) needed at Bridgeport Healthcare Center to lead accurate and timely MDS assessments and coordinate the RAI process. Responsibilities include providing Medicare/Medicaid (case mix) oversight, ensuring appropriate clinical services and reimbursement, ensuring compliance with federal/state regulations and company guidelines, and serving as a key member of the facility’s management team to support quality outcomes. Qualifications include graduation from an accredited nursing program and active RN license in the employing state; three years of long-term care experience and experience with MDS/RAI or case management preferred.
