Job Summary
Direct day-to-day MDS Office functions by conducting resident assessments on admissions and during significant changes, formulating and monitoring individualized care plans, and completing weekly reports and medical forms. Interpret department policies for residents, staff, and government agencies while admitting, transferring, and discharging residents as required. Ensure MDS guidelines and infection control procedures are followed during surveys, quality assurance reviews, and standard of care meetings. Maintain accurate Medicare binders, census data, and accident/incident reports, providing training to Direct Care staff on documentation processes.
Required Qualifications
- Graduate of an approved RN program
- Current license status
- Ability to sit, stand, bend, lift and move intermittently during working hours
- Willingness to work beyond normal working hours
- Willingness to work in other positions temporarily when necessary
- Ability to communicate with medical staff, nursing personnel, and other department supervisors
- Ability to maintain a liaison with residents, their families, support departments, etc.
Desired Qualifications
- Current experience as an MDS Coordinator
- Recent formal training as an MDS Coordinator
