RN MDS Coordinator

Job Summary
Conduct and coordinate comprehensive resident assessments (MDS) and Care Area Assessments (CAAs) in accordance with federal, state, and facility regulations. Ensure timely completion of assessments and care plans while collaborating with interdisciplinary teams to develop individualized resident care plans. Monitor assessment schedules to maintain compliance with Medicare and Medicaid guidelines, review clinical documentation for accurate coding and reimbursement, and assist with surveys, audits, and quality improvement initiatives. Serve as a clinical resource for nursing staff regarding assessment and documentation requirements. Maintain current knowledge of MDS regulations and reimbursement processes.
Required Qualifications
- Nursing degree from an accredited college or university
- Current, active, and unencumbered Registered Nurse (RN) license in the state of practice
- Minimum of one (1) year of experience in a hospital, long-term care facility, or related healthcare setting
- Experience completing and coordinating MDS assessments in a long-term care or skilled nursing facility
- Strong organizational, analytical, and communication skills
- Knowledge of federal and state regulations related to resident assessments and care planning
Desired Qualifications
- RAC-CT certification or advanced MDS training
- Experience with Medicare, Medicaid, and skilled nursing reimbursement processes
- Previous experience in long-term care clinical leadership or care coordination roles
