Job Summary
Coordinate the planning, organizing, directing, and managing of the Minimum Data Set (MDS) and Case Management System to assure clinical information submitted is in compliance with federal, state, and tribal requirements. Schedule and oversee resident MDS assessments, Care Plans, and CAAS submissions to CMS while maintaining audit duties, filing, and computer data entry. Actively work with case managers, physicians, and ancillary providers to verify Quality Indicators, print monthly reports, and follow up on action plans. Maintain liaison with internal and external departments, agencies, and community resources to determine appropriate resource utilization. Recognize and inform the supervisor of unusual nursing care problems, participate in QA and Performance Improvement projects, and prepare designated monthly reports. Adhere strictly to confidentiality policies regarding patient files and division memos while demonstrating dependability and punctuality in this office-based role at Tsali Care Center.
Required Qualifications
- Current license as a Registered Nurse in North Carolina
- Graduation from an accredited school of nursing
- Minimum one year nursing experience
- At least six months MDS/Care Planning and long-term care experience
- 3 years of work-related experience in Long Term Care
- Valid NC driver's license
- Program proficiency upon hire
- Knowledge of the MDS/Care Plan Process
- Thorough knowledge of the office procedures, policies and guidelines of the Cherokee Indian Hospital, Health and Medical Division and the Tsali Care Center Services Program
- Thorough knowledge of the 3.0 MDS system software Point Click Care
- Proficiency in the use of Microsoft Windows, Excel Spreadsheets, Word
- Ability to write routine reports and correspondence effectively
- Ability to organize work, deal effectively with the public, communicate effectively, both verbally and in writing, follow instructions, and work independently or as a member of a group
- Ability to maintain medical records
- Ability to collate and compile data; and prepare reports and summaries
- Practical knowledge of the facilities organization and services, the basic rules and regulations governing visitors and resident treatment, practical knowledge of standard procedures, medical records, and medical terminology
- Ability to legibly document and maintain clinical records, files, reports and logs
- Understanding of the EBCI culture, lifestyles and risk factors
- Tact, courtesy, and professional conduct
- Cultural sensitivity to residents' needs
- Strict adherence to all applicable tribal confidentiality policies, procedures and guidelines
- Close attention to detail and mental concentration
- Ability to lift 25 pounds and carry 5 pounds on a repetitive basis
