Job Summary
RN Manager - Care Management Social Work leads the inpatient Care Management Department at Providence Portland Medical Center. The role directs and manages a team of approximately 24-31 FTE, with a budget of approximately $2.4-2.9M, and oversees care management operations for the facility. Responsibilities include providing leadership for the clinical care management model, promoting team functioning, advancing care coordination, and collaborating with Medical Staff, Utilization Management, Finance, and community/government partners to establish effective discharge planning and transitions to home and community settings. The CM Manager acts as a strategic partner to Oregon PH&S facilities and oversees programs such as Patient Assistance Funds. The position requires a Bachelor's Degree in Nursing, an Oregon RN license, 2 years of supervisory experience, 1 year of care management experience, and 3 years of relevant experience in Social Services, Utilization Review/Quality Management, or Case Management. Preferred qualifications include a Master’s Degree in Nursing. The role is full-time with day shift and offers a comprehensive benefits package.
Required Qualifications
- Bachelor's Degree in Nursing
- Oregon Registered Nurse License upon hire
- 2 years of Supervisory experience, including program planning, performance management, staff development and evaluation
- 1 year of Care Management experience
- 3 years of Relevant experience in Social Services, Utilization Review/Quality Management, or Case Management
