Job Summary
RN Manager - Care Management Social Work leads the inpatient Care Management Department, directing a staff of approximately 24-31 FTE with a budget of roughly $2.4–$2.9M. Oversees the clinical care management model, coordinates discharge planning and transitions to home/community, collaborates with Medical Staff, Utilization Management, Finance, and community/government partners, and serves as a strategic partner to other Oregon PH&S facilities to meet the Triple Aim.
Required Qualifications
- Bachelor's Degree in Nursing
- Oregon Registered Nurse License upon hire
- 2 years of Supervisory experience, including program planning, performance management, staff development and evaluation
- 1 year of Care Management experience
- 3 years of Relevant experience in one or more of the following areas: Social Services, Utilization Review/Quality Management, Case Management
