Job Summary
RN Manager - Care Management Social Work leads the inpatient Care Management Department, directing the clinical care management model and overseeing a staff of approximately 24-31 FTE with a budget of about $2.4-2.9M. Collaborates with Medical Staff, Utilization Management, Finance and community partners to optimize discharge planning and transitions to home and community settings. The role serves as a strategic partner to PH&S facilities, supports a culture of patient-centered care, and ensures coordination across disciplines. Requires a Bachelor’s Degree in Nursing and an Oregon RN license with supervisory and care management experience; preferred Master’s Degree in Nursing.
Required Qualifications
- Bachelor's Degree in Nursing
- Oregon Registered Nurse License upon hire
- 2 years of Supervisory experience, including program planning, performance management, staff development and evaluation
- 1 year of Care Management experience
- 3 years of Relevant experience in one or more of the following areas: Social Services, Utilization Review/Quality Management, Case Management
