Job Summary
RN Manager - Care Management Social Work at Providence Portland Medical Center leads the inpatient Care Management Department (staff 24-31 FTE, budget ~$2.4–$2.9M) and directs the clinical care management model. Provides leadership to advance care coordination and discharge planning, collaborating with Medical Staff, Utilization Management, Finance and community partners to optimize patient care and transition planning, and serves as a strategic partner to operate across Oregon PH&S facilities to achieve the Triple Aim. Key responsibilities include directing department operations, promoting team functioning, and overseeing the Patient Assistance Fund programs within the facility, with a focus on leadership, coordination of care, and cross-disciplinary collaboration.
Required Qualifications
- Bachelor's Degree in Nursing
- Oregon Registered Nurse License upon hire
- 2 years of Supervisory experience, including program planning, performance management, staff development and evaluation
- 1 year of Care Management experience
- 3 years of Relevant experience in one or more of the following areas: Social Services, Utilization Review/Quality Management, Case Management
