Job Summary
RN Manager in Care Management Social Work leads the inpatient Care Management Department at Providence Portland Medical Center; oversees a team of ~24-31 FTE, budgeting, and coordination of patient care, discharge planning, and transition of patients back to home/community, collaborating with Medical Staff, Utilization Management, Finance, and external partners to advance the clinical care management model and Triple Aim initiatives. Requires RN license in Oregon, Bachelor’s in Nursing, and 2 years supervisory plus 1 year care management experience; Master’s in Nursing preferred. Day shift, full-time with incentive programs and comprehensive benefits.
Required Qualifications
- Bachelor’s Degree in Nursing
- Oregon Registered Nurse License
- 2 years of Supervisory experience, including program planning, performance management, staff development and evaluation.
- 1 year of Care Management experience.
- 3 years of Relevant experience in one or more of the following areas: Social Services, Utilization Review/Quality Management, Case Management.
