RN Manager, Care Management

Supervise and direct the Care Management team, including RN Care Managers, Coordinators, and Discharge Planners, while overseeing clinical supervision, scheduling, and Kronos payroll. Manage post-acute provider networks, assess patient care levels, and collaborate with Social Work and ED teams to ensure seamless transitions and reduce readmissions. Initiate, review, and implement Care Management policies, oversee the electronic referral system, and establish training programs for staff and providers. Monitor management metrics, handle grievances, and participate in multidisciplinary conferences to support the Population Health Program.
