RN Manager, Care Management

Supervise and direct all levels of the Care Management team, including RN Care Managers, Coordinators, and Discharge Planners, while overseeing clinical supervision, scheduling, and Kronos payroll. Manage post-acute provider networks, vet new facilities, and ensure seamless patient transitions from MLK entities to community healthcare providers. Initiate, review, and implement Care Management policies to reduce readmissions and improve community health, while collaborating with Social Work, ED, and Population Health teams. Establish staff training programs, monitor metrics for quality and outcomes, and address customer grievances. Maintain compliance with ACMA & CMSA standards and participate in multidisciplinary conferences.
