RN Manager - Care Management Full-time Day

RN Manager – Case Management leads collaborative care coordination and case management activities in an acute-care setting. Responsible for assessment, planning/intervention, implementation, care coordination, monitoring, and evaluation of the plan of care, working with an interdisciplinary team to ensure patient needs are met in the community. Focuses on optimizing health outcomes, advocacy, appropriate utilization of resources, and collaboration with post-acute care providers. Day shift, Full-time position at Petaluma Valley Hospital, Petaluma, CA. Requires a Bachelor’s Degree in Nursing and CA RN license; 5 years healthcare experience, with 3 in Case Management and 1 year in leadership. Preferred CCM/ACM certification and Master’s degree in Healthcare or related field.
