Job Summary
Conduct daily clinical operations on the unit, ensuring high-quality resident care, regulatory compliance, and effective team coordination. Provide individualized attention including assistance with grooming, bathing, feeding, incontinent care, toileting, and administering medications. Monitor residents for safety risks such as falls, skin breakdown, and changes in behavior, while promptly reporting observations to licensed nursing personnel. Ensure accurate, timely documentation in the EMR, complete required assessments, and support safe discharge planning in collaboration with the care team. Maintain a safe, neat environment by adhering to infection control procedures and reporting environmental deficiencies. Answer client call lights promptly and communicate unit updates to the NP and DON.
Required Qualifications
- Active RN or LPN license in the state of Illinois
- Ability to frequently push, pull, move 20-30 lbs.
- Ability to occasionally lift a minimum of 50 pounds
- Must be able to sit, stand, bend, lift, walk and climb stairs, as needed
- Must be able to read, write, speak, and understand the English language
- Must be able to cope with the mental and emotional stress of the position
- Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility
- Must stay on unit until relieved by oncoming staff
- Must meet the general health requirements set forth by the policies of this facility
- Aptitude for detail work
- Ability to make independent decisions when circumstances information includes, but shall not be limited to, information concerning Cantata/British Home former and current clients/clients, medical and health information and Cantata/British Home information systems including but not limited to the Internet and E-mail
- Must possess the ability to deal tactfully with personnel, clients/patients, family members, visitors, government agencies/personnel and the general public
- Must possess the ability to communicate effectively both verbally and in written form with a diverse group of people
- Ability and willingness to work harmoniously within a team environment departmentally
- Excellent communication, organization, and leadership skills
- Strong knowledge of long-term care regulations (IDPH/CMS)
- Must be able to frequently push, pull, move 20-30 lbs., and occasionally lift a minimum of 50 pounds
- Sits, stands, bends, lifts, walks and climbs stairs, as needed to perform tasks/duties
- May be scheduled to work weekends and holidays based on operational needs
- Schedules are subject to change based on operational need

- Must stay on unit until relieved by oncoming staff. If oncoming staff is late or missing inform the scheduler/DON
Desired Qualifications
- Clinical experience in skilled nursing or acute care (preferred)
