Job Summary
Provide total nursing care to residents on the assigned unit during the shift, administering medications and treatments as prescribed while evaluating their effectiveness. Participate in developing individualized care plans with the interdisciplinary team, educate residents and families regarding procedures, and monitor team member performance for coaching and corrective action. Delegate tasks appropriately, recognize and respond to significant changes in resident condition, and accurately document care in accordance with facility policies. Serve as Charge Nurse by answering questions and supporting clinical decision-making. Remain current on infection prevention practices and regulatory requirements.
Required Qualifications
- Graduate of an accredited nursing education program
- Active Illinois Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in good standing
- No prior abuse convictions
- Ability to follow facility policies, maintain resident safety, and deliver compassionate, resident-centered care
- Ability to work effectively with diverse resident populations, including elderly and cognitively impaired individuals
- Ability to perform the essential functions of the position with or without reasonable accommodation
Desired Qualifications
- Previous skilled nursing, rehabilitation, or long-term care experience preferred
