RN/LPN - Per Diem Overnights

Conduct direct resident care and administer medications in accordance with facility policies, OBRA, and HIPAA standards. Collaborate with interdisciplinary teams to formulate and update individualized care plans, participating in resident care plan meetings. Institute protocols for falls prevention, behavior management, infection control, and pain assessment while assessing and documenting changes in residents' status. Provide leadership by preparing daily assignments, delegating workload, and supervising direct caregivers through observation. Obtain, inventory narcotics, and maintain positive relationships with team members, residents, and families to facilitate open communication.
